ED treatments, explained

A plain-English tour of every mainstream option — what works, how it works, and when to escalate.

The good news: ED is one of the most treatable conditions in medicine. Most men respond to first-line options, and there’s a clear ladder if the first step isn’t enough.

First-line: PDE5 inhibitors

Sildenafil (Viagra), tadalafil (Cialis), and similar prescription pills are the standard first treatment. They enhance the natural nitric-oxide signal so blood can fill the penis when you’re aroused — they don’t create desire on their own. They’re effective for most men and now widely available generically.

Treat the root cause first

Pills manage the symptom. If low testosterone, blood sugar, blood pressure, or sleep apnea is driving your ED, treating that can improve results — and your overall health. Start by knowing your numbers.

Test, don’t guess

Low testosterone is a common, fixable cause of ED. Check your levels from home with a physician-reviewed hormone panel before spending a dime on pills.

Vacuum erection devices

A vacuum pump draws blood into the penis and a ring holds it there. Non-drug, inexpensive, and effective for many men — useful if you can’t take or don’t want medication.

Injections and suppositories

When pills don’t work, injectable medication (alprostadil) or urethral suppositories are highly effective second-line options prescribed by a physician.

Emerging: shockwave therapy

Low-intensity shockwave therapy aims to stimulate new blood-vessel growth. Early results are promising for some men, but it’s still considered investigational — approach clinics making big promises with healthy skepticism.

When ED is urgent

Seek prompt care if ED comes on suddenly, follows pelvic injury or surgery, or comes with chest pain. And an erection lasting more than four hours (priapism) is a medical emergency.

The bottom line

Start with lifestyle and testing, use first-line pills when appropriate, and escalate methodically if needed. Nearly every man can find an option that works — often without ever setting foot in a clinic.
